Forms to sign
All new patients must fill out these forms (either digitally or printed) before medical services can be provided. New patients have three options:
You may print out the forms, fill out and sign them, and bring them with you to your first appointment
You may download them, fill them out digitally, and send them to sdansereau@dulaspringswellness.com
Or you may arrive a little early to your appointment and I will provide you with the forms to fill out and sign to fill out the forms in the office.
